Mission Statement
|
The Imani Health Education and Outreach Program strives to provide health education and services to meet the needs of Oak Park community members facing chronic illnesses, such as diabetes and high blood pressure (hypertension). We believe providing proper health education and resources that will allow for increased awareness will improve outcomes and prevent chronic illness. In addition, we hope to provide counseling and personalized lifestyle change plans to further connect with individuals within the Oak Park community.

What do we do?
Our Imani Clinic undergraduate volunteers train under the guidance and mentorship of our Co-Director medical students and PA students to create:
- Thoughtful and proactive discussion with Oak Park Community Members through one-on-one lifestyle change sessions inside Imani Clinic.
- In addition, we provide resources for understanding and navigating through several diseases, such as hypertension and diabetes.

One-on-one health education and lifestyle change sessions at Imani Clinic are available by walk-in or appointment every Saturday.
